3. Incentives are in place through December 18, 2026. Funds are limited so submit required documentation as soon as possible.
4. Required documentation must be submitted within 3 months of purchase date.

6. Required documentation listed below must be submitted no later than 3 months after purchase date.This incentive form
   1. A copy of your receipt or invoice for each item with purchase price(s) circled
   2. For fixtures include packaging or documentation showing number of lumens

If all required information AND documentation are not provided, rebate will be rejected immediately.

LED Bulb - $0.50/bulb (Quantities less than 5 bulbs do not qualify)
LED Exit Sign - $5/sign
LED Fixture - $0.50 per 800 lumens
Occupancy Sensor - $5/sensor (Does not include: Motion detector bulbs or fixtures)
